Stacey McNeely maintains a professional interest in transitional care and the reduction of readmission rates for elderly patients.

Transitional care is an increasingly important aspect of care for patients who were recently released from the hospital. The period of transitional care is vital for ensuring the continued recovery of the patient, and for reducing the incidents of readmission to a hospital. Hospital readmissions cause increased medical costs, but transitional care can help to reduce that extra expense.

Around one in five Medicare beneficiaries gets readmitted to the hospital within 30 days of their release. Experts believe that more than two thirds of these cases are preventable with the proper follow-up and assistance with new treatments or medications after release. The main goal of transitional care management is to reduce the readmission rate for these patients and provide them with the necessary support to ensure complete recovery. Programs like Able Care Connect provide a service that is billable to Medicare. This particular program is aimed at supporting acute care patients for the 7 days following their discharge.
